Appointment Policy:

Cornerstone Veterinary Hospital staff members treat every patient with the utmost love and respect. We work by appointment. In an effort to accommodate requests for same-day appointments, we require a phone call, email, text, or app cancellation 24 hours prior to your appointment time if you are unable to keep your scheduled appointment. Upon re-scheduling the appointment, we will collect a pre-payment for the exam that will go towards your next appointment.

We reserve the right to keep the pre-paid office visit fee or technician exam fee on the second missed appointment, without adequate notice (24 hours), in a 365-day window. This pre-payment collection will be reapplied for every subsequent missed appointment per scheduled pet. Appointments containing multiple pets are required to pre-pay a minimum of the office visit fee or technician exam fee upon scheduling due to the difficulty in filling multiple time slots in a short time frame.

We reserve the right to keep part or all of the pre-paid amount for a minimum of one time slot up to the maximum amount of time slots reserved that we are unable to fill. An extended exam fee may be incurred when additional services are requested beyond what was initially scheduled for the pet. There is no guarantee an additional service request may be accommodated, and staff may ask to schedule an appointment at a later time to address the request.

Authorization of Care:

I hereby certify that I am the owner, or a duly authorized agent of the owner, for all animals listed on the above-described account. I authorize Cornerstone Veterinary Hospital veterinarians & staff to examine, prescribe for, and/or treat all animals listed on the above-described account. I assume responsibility for all charges incurred in the care of all animals on the abovedescribed account. I also understand that any and all charges will be paid for at the time when services are rendered and that a deposit may be required for hospitalization, treatment and/or surgery.
